Old feedback becomes the current story

Ollie asks Otto to prepare talking points for Dana using earlier 1:1 notes. Otto repeats a concern from an old project but leaves out the later note saying it was resolved. Dana sees a current problem instead of a completed change, and the new conversation starts from a story that is already out of date.

Why it looked good

A recurring phrase is easy to extract from old notes. Its frequency does not tell the reader whether the concern is still open.

Q1 · What is this really for?

Should these talking points explain a current concern or review how the earlier change worked?

Q2 · Who decides, and did they?

Who can confirm the concern's current status, and have Dana and Ollie checked the later resolution together?

The next move

Ollie includes the resolution beside the old concern and asks Dana which current example they should discuss, if any.
