ask two questions

Who decides what?

One page for work that several people touch. For each decision, write who decides, who is consulted first, who is told after and who does the work, then copy a plain-text summary.

Every example on this page is fictional. What you type stays on this page: nothing is saved, sent or stored, and there are no cookies. Print gives you the filled sheet on one page.

Step 1. The work
Step 2. Decisions

One box per decision, written as a sentence: what gets decided (a date, a scope, an access, a record). Split a decision if two things are decided. Decides holds one name: the person who can say yes and have it hold. If you do not know, write Unknown. Never write two.

Decision 6
Step 3. Not deciders

Write any tool or person who can describe the work but cannot approve it, and what they cannot approve.

Step 4. Rules check

Check each line only when it is true. An empty box is the next thing to fix.

Step 5. Gaps to close

Each gap becomes a question. One per line.

Your summary

It is built from the sheet above and updates as you type. Read it once and change anything before you share it.

Consulted is input, not approval: only the name in Decides decides. This sheet is practice material, not advice about a real situation. It is template 5 of the template pack; the PDF version is in the template pack v2 add-on.
